Passenger locator forms (during the coronavirus pandemic)

If you are traveling to the UK from Turkey you will need to fill out a passenger locator form.

You must complete this form before you arrive in the UK. You cannot submit the form until 48 hours before you’re due to arrive in the UK.

You’ll need to show your completed form when you arrive at the UK border, either by printing a copy, or showing it on your phone.

The government will use this information to contact you if you or someone you’ve travelled with develops coronavirus (COVID-19) symptoms.

If you need to self-isolate for the first 14 days after you arrive in the UK, the government will also use the information to check that you’re self-isolating.

What to do when arriving into Ireland

COVID-19 Passenger Locator Form

You must fill in a form before you arrive in Ireland called the COVID-19 Passenger Locator Form. Failure to do so is an offence.

The form may be used by health authorities to contact you in order to verify your location in the country.

It will also help contact tracers get in touch with you should someone on your flight or ferry be confirmed as having COVID-19.

If you are travelling to Turkey, you will also need to fill out a passenger locator form. Depending on the airline, you may get an email where you fill out the form before travel or you will fill it out during the flight.
